The Croatia visa application form must be completed through the official online system provided by the Croatian Consulate. After logging into the system, you must select “New Application” to begin. The form can be filled out either in Turkish or English.

  • Sections 1–11: Contain your personal and identity information.
  • Sections 12–16: Contain your passport details. Be careful—writing your passport number incorrectly may directly result in visa refusal.
  • Occupational details: Employees, employers, students, and retirees must provide information appropriate to their status. Students must provide the name and contact information of their school, while retirees only need to indicate “retired.”
  • Travel details: Travel dates, previous visas, fingerprint records (if any), and Schengen visa codes must be declared. Travel dates must match other documents such as your flight, hotel booking, invitation letter, and travel insurance.
  • Accommodation and expenses: The form must include details of who will cover your expenses, as well as accommodation and invitation details.
  • Relatives in EU/EFTA/Switzerland: If you have relatives in these countries, their details must be declared. Failure to do so may give the impression of withheld information.
  • Signature section: The application date, place, and applicant’s signature must not be left blank. For minors, parents or legal guardians may sign on their behalf.
  • Additional notes: Read and acknowledge the additional information provided by the consulates at the end of the form, then add the date, place, and your signature.

If someone else will submit your application on your behalf, you must issue a notarized power of attorney authorizing them. This document must contain both your personal details and those of the representative.

  • The form must be completed online. Handwritten or unsent forms are not valid.
  • Since the form is directly linked to your supporting documents, all information must be complete and accurate.
  • Your email address and phone number must be active and reachable. In business and visit visa applications, the inviter’s contact details must also be valid.
  • The consulate may request additional information or documents if necessary.
